Documents needed to monitor the validity

  1. Identification Card (Father’s name, social security number or OIB)
  2. Certificate
  3. Medical evidence
  4. Seaman’s book (matricula)

Documents required for tax reporting

  1. National ID (identification and OIB)
  2. Employer’s confirmation of paid receipts (payroll)
  3. An excerpt from seeman book, or permit boarding, as evidence of the number of days spent on board in international waters.
  4. Print the official data of the Tax Administration – the state of paid contributions during the year (tax card)
  5. Confirmation of dependents (children, spouse, parents, if they have no income, their ID number or OIB)
  6. Bank certificate of payment of life insurance premiums
  7. Accounts of medical examinations and orthopedic appliances (and dependents)
  8. Other confirmation of the possible other receipts (contract for services, royalties, renting an apartment, renting a cottage, membership in sports society, etc.)
  9. Certificates of renovating the apartment (Bills)
